Procarbazine: From Hodgkin Lymphoma to Follicular Lymphoma
One-Sentence Summary
Procarbazine is an oral alkylating-type chemotherapy agent, originally used to treat Hodgkin lymphoma. The TxGNN model predicts it may be effective for follicular lymphoma, but the supporting evidence is limited: 2 related clinical trials (neither directly tests procarbazine) and 12 publications, mostly retrospective or regimen-level reports.

Why is This Prediction Reasonable?
Currently, detailed mechanism of action data is not available. Based on known information, procarbazine is an oral alkylating-type cytotoxic agent. Its efficacy in Hodgkin lymphoma is established, and mechanistically it may be applicable to other lymphoid malignancies such as follicular lymphoma.
Both Hodgkin lymphoma and follicular lymphoma are lymphoid cancers that respond to alkylating chemotherapy. The literature includes a case report of two patients with relapsed or refractory follicular lymphoma who achieved a complete and durable remission with prolonged daily procarbazine (PMID 16690522). A metronomic low-dose oral regimen containing procarbazine (PEP-C) has also been reported in relapsed or refractory lymphoma.
However, the evidence is largely retrospective or at the regimen level, so the specific contribution of procarbazine cannot be isolated. Procarbazine-containing regimens also fell out of favour in non-Hodgkin lymphoma with the arrival of CHOP.
Clinical Trial Evidence
| Trial Number | Phase | Status | Enrollment | Key Findings |
|---|---|---|---|---|
| NCT00003113 | Phase 2 | Terminated | 6 | Oral combination chemotherapy with G-CSF in elderly patients with intermediate- and high-grade non-Hodgkin lymphoma. The regimen plausibly includes procarbazine, but only 6 participants were enrolled, so there is essentially no interpretable efficacy data. |
| NCT00577993 | Phase 3 | Completed | 210 | Fludarabine, mitoxantrone and dexamethasone plus rituximab in stage IV indolent lymphoma, comparing concurrent versus sequential rituximab. The regimen does not appear to include procarbazine, so it gives disease context only, not direct evidence. |

Cytotoxicity
| Item | Content |
|---|---|
| Cytotoxicity Classification | Conventional cytotoxic (alkylating-type agent) |
| Myelosuppression Risk | High (the knowledge graph lists leukopenia, thrombocytopenia and anaemia as contraindication-related conditions) |
| Emetogenicity Classification | Please refer to the package insert warnings and precautions |
| Monitoring Items | CBC with differential and platelets, liver and renal function |
| Handling Protection | Must follow cytotoxic drug handling regulations |
Safety Considerations
- Conditions flagged in the knowledge graph (not label text): liver failure and liver disease, kidney disease, gallbladder disease, thrombocytopenia, leukopenia, anaemia (including macrocytic anaemia), blood coagulation disease, peripheral neuropathy and peripheral nervous system disease, infectious disease (including fungal infection and dermatophytosis), pyoureter, and substance abuse or dependence.
- Drug interactions: 100 interactions were found. Major ones among those reviewed include isometheptene, bupropion, morphine, lorcaserin, diethylpropion, phentermine, fenfluramine, dexfenfluramine, mazindol, ephedrine, and the 5-HT3 antagonists ondansetron, granisetron, palonosetron and dolasetron. Moderate ones include epinephrine (including topical and ophthalmic forms), nasal ephedrine and metoclopramide. Levofloxacin is a minor interaction.
- Other literature signals: Procarbazine produced tumours in rodent carcinogenicity bioassays, and procarbazine-containing regimens are associated with gonadal damage and infertility in survivors.
Please also refer to the package insert for full warnings and contraindications.
Conclusion and Next Steps
Decision: Hold (research question)
Rationale: Biological plausibility is reasonable, and there are case-level reports of responses in relapsed follicular lymphoma. However, no trial tests procarbazine directly, the evidence cannot isolate its contribution, and procarbazine has no valid New Zealand authorization. The safety profile (myelosuppression, carcinogenicity, fertility effects, many major interactions) calls for caution.
To proceed, the following is needed:
- Package insert warnings and contraindications (Medsafe)
- Mechanism of action data (DrugBank)
- Prospective data on procarbazine-containing regimens in follicular lymphoma, compared with current standard therapy
- A regulatory pathway, since the only New Zealand authorization has been cancelled
